I intentionally design my templates to be as inclusive as possible while keeping documentation neutral and clinically sound. Instead of "parental history," I use "Guardian 1-3" to reflect diverse family structures. I don’t include specific references to sexuality, religion, or culture due to our current political climate, ensuring documentation remains focused on care without unnecessary risk.
For therapists working with LGBTQIA+, undocumented, minority, and at-risk clients, I strongly encourage using standard language across all documentation to maintain privacy and safety. This includes using "partner" instead of gendered terms, initials instead of full names when possible, and they/them pronouns as a default when gender identity isn’t relevant to clinical care.
Your notes should reflect professional, client-centered care while avoiding details that could be misused. I urge all therapists to prioritize their clients' safety by keeping documentation neutral, factual, and free from unnecessary identifiers.
